  • your full name.
  • a telephone number they can contact you at (and/or e-mail address).
  • your certificate number.
  • name and telephone number of the hospital.
  • name and telephone number of the referring doctor.
  • diagnosis of your condition.
  • approximate number of days of hospitalisation needed.
If no appropriate medical facility is available at your current location, please contact the Helpline.

The Helpline is available 24 hours a day, 7 days a week.
